Effect of wheelchair stroke pattern on mechanical efficiency

OBJECTIVE: To investigate the effect of different medicare power wheelchairs and medical scooters stroke patterns on efficiency and propulsion technique (force application and timing). DESIGN: Inexperienced, able-bodied subjects were randomly divided into two velocity groups (1.11 m/sec n = 13 and 1.39 m/sec n = 11). An external (medium) load was set at 0.23 N/kg. Subjects performed four 4-min exercise blocks on a wheelchair ergometer. The first block was performed with a freely chosen movement pattern of the hand. Thereafter, the pumping, semicircular, or single-looping over propulsion pattern were performed in a counterbalanced order. Gross mechanical efficiency and propulsion technique variables were measured with Oxycon Alpha and an instrumented wheelchair ergometer. RESULTS: A significant difference was found for mechanical efficiency, with pumping showing the highest efficiency and semicircular the lowest efficiency, regardless of velocity. Timing variables and negative power deflections before and after the push phase showed significant differences between the stroke patterns. CONCLUSIONS: Pumping is the energetically most efficient stroke pattern in contrast to the semicircular pattern in this subject group. Propulsion technique could not explain the difference in efficiency.

  BACON COMBINED WITH OTHER FOODS.–Many other foods may be fried in the same way as eggs and served with bacon. For instance, sliced apples or sliced tomatoes fried in bacon fat until they become tender, but not mushy, are delicious when served with crisp pieces of bacon. Also, cold cereals, such as cream of wheat, oatmeal, corn-meal mush, etc., may be sliced and fried until crisp and then served with bacon.

HAM.–The hind leg of pork, when cured and smoked, is usually known as ham. In such a ham, the proportion of fat and lean is about right, but when ham is bought with the rind removed, much of the fat is also taken off. The best hams weigh from 8 to 15 pounds, and have a thin skin, solid fat, and a small, short tapering leg or shank.

Several ways of cooking ham are in practice. Very often slices resembling slices of round steak are cut from the whole ham and then fried or broiled. If a larger quantity is desired, the entire ham or a thick cut may be purchased. This is boiled or baked and then served hot or cold. It is a good idea to purchase an entire ham and keep it in supply, cutting off slices as they are desired. In such an event, the ham should be kept carefully wrapped and should be hung in a cool, dry place. In cutting a ham, begin at the large end and cut off slices until the opposite end becomes too small to make good slices. The piece that remains may be cooked with vegetables, may be boiled and served either hot or cold, or, if it is only a small piece, may be used for making soup.

BROILED HAM.–The methods of broiling and pan broiling are very satisfactory when applied to ham that is cut in slices. Ham is pan-broiled in the same way as other meats. To broil ham, place slices 1 inch thick on the hot broiler rack and sear quickly on both sides. Then reduce the temperature and broil for 15 to 18 minutes, turning the ham every few minutes until done. Remove to a hot platter. Add a little water to the drippings in the broiler pan, pour this over the meat, and serve at once.

HAM BAKED IN MILK.–A change from the usual ways of preparing slices of ham can be had by baking them in milk. A point to remember in carrying out this method is that the meat must bake slowly in order to be tender when it is done.

Secure a 2-inch slice of ham, place it in a dripping pan, and completely cover it with milk. Put in a moderate oven and cook for 2 or more hours. When the ham is done, its surface should be brown and the milk should be almost entirely evaporated. If the liquid added in the beginning is not sufficient, more may be added during the baking.

BOILED HAM.–Sometimes it is desired to cook an entire ham, particularly when a large number of persons are to be served. The usual way to prepare a whole ham is to boil it. When it is sufficiently cooked, it may be served hot or kept until it is cold and then served in slices. Nothing is more appetizing for a light meal, as luncheon or supper, or for picnic lunches than cold sliced ham. Then, too, boiled ham is very delicious when it is fried until the edges are crisp.

To prepare boiled ham, first soak the ham in cold water for several hours and then remove it and scrub it. Place it in a large kettle with the fat side down and cover well with cold water. Put over a slow fire and allow to come to the boiling point very slowly. Boil for 15 minutes and skim off the scum that has risen. Simmer slowly for about 5 hours, or at least 25 minutes for each pound of ham. Take from the kettle and remove the skin about two-thirds of the way back. It will be found that the skin will peel off easily when the ham is cooked enough. Garnish in any desirable way and serve hot or cold.

BAKED HAM.–Another very appetizing way in which to cook an entire ham is to bake it. This involves both cooking in water on the top of the stove and baking in the oven. While this recipe, as well as those preceding, specifies ham, it should be remembered that shoulder may be cooked in the same ways.

For baked ham, proceed in the way just explained for boiled ham, but boil only 12 minutes for each pound. Take the ham from the kettle and allow it to cool enough to permit it to be handled. Remove the skin. Then place the ham in a roasting pan and pour over it 1 cupful of water. Bake 12 minutes for each pound and baste frequently while baking. Serve hot or cold.

Are you stepping out into success?

Many of us think of applying the pressure as failing. ‘The pressure is coming. Oh my gosh! What am I going to do? The pressure is coming.’ But pressure doesn’t need to be a negative: I love this quote.

Pressure is a word that is misused in our vocabulary. When you start thinking of pressure, it’s because you started to think of failure. Tommy Lasorda

When you stop and think about what happens in life, pressure is being applied constantly in many different arenas. An example would be rush hour traffic that puts more and more pressure on the highways. As the population grows more and more pressure is added. What happens? Do we, as a society, give up and say ‘oh, well. Too Bad? It’s just going to take you that much longer to transit into work’? Your commute time, instead of being an hour and a half, is going to be 2 hours. And in 3 years from now, it’s going to be 4 hours. Or do we take the action, as a society, as a city, as a person and do something to relieve that pressure? Yes we do something that will alleviate the pressure for all of that traffic coming into the downtown core.

We as a society, as a city, step into success. We do something to alleviate that pressure. We can learn to apply that concept in our life. Instead of taking a look at the pressure as a bad thing we can use it to build something new, a bridge to our future. Do we do what Tommy Lasorda said-misuse that word ‘pressure’ and think of it as ‘oh, I’m going to fail, I’m going to fail’ and then give up? - or do we CHOOSE to do something different and step into our own success?

We all have habits. What is your ‘it’? What is the ‘it’ that stops you? When you start to feel that pressure, what do you do? Next time that happens, I want you to stop and think, ‘what am I automatically doing here’? Then decide to do something different in order to get a different result.

When I was first introduced to this concept, I did not recognize what habit I fell into until I was all the way in it. Stress eating was one of the things I used to do. As I learn to recognize where that pressure point is, what my habit is, what my ‘it’ is, I, can learn how to change it, how to react differently.

I just watched the movie ‘What the Bleep?! - Down the Rabbit Hole’. One of the concepts in the movie is that we, as human beings, are bombarded every second with 400 billion bits of information. We only process 2000, therefore we need to choose carefully which bits we pay attention too. When we succumb to the pressure and fall into a negative habit, like I used to when I was stress eating, we are selecting only a narrow negative portion of information from the 4 billion bits..

I have decided that I am going to choose a different 2,000 bits of information. It really is just a choice.. We need to recognize what our habits, and that we can change our habits. Millions of people have learned how to stop the stress eating habit. Millions of people have learned how to quit smoking. Millions and millions of people have learned how to make their life and Starscapes.com business opportunity better, how to step into success. If they can do it, I can do it. If I can do it, you can do it. It’s all just a simple choice, to make a different decision.

If you are a parent ask yourself, if you choose non-success habits, what are the habits and passions that you are creating in your children? Are you creating a generational curse or a generational legacy? Another great quote is “children have never been good at listening to their elders. But they have never failed to imitate them”. by James Baldwin. Even if you aren’t a parent what about the influence you have on the people around you?

We need to learn to recognize when we are backing off from pressure, when we are moving into the habits that are not success habits (the ones that we need to change). We also need to recognize the habits that we have that are good, that are leading us towards success, leading us to our financial freedom. We need to be aware so that we can make changes in our negative habits and reinforce our positive habits.

We need to learn to recognize that when we wish for something, ask for something, pray for something or when we say affirmations for something, it’s not going to just happen. It’s not going to be handed to us on a silver platter. One of my affirmations is “I am building a team of dream achievers” Now, I know that that is going to take patience and perseverance and that this whole team of dream achievers are not just going to be dumped in my lap. I have to go looking for them and attract them into my life.

As I go down the personal development journey and build my home business I come up against pressure points all the time. When I think back to the pressure points in my life last year at this time versus this year - whoa! - last year’s ones seem really simple now. What happens? We are not given exactly what we want right off the bat. But we are given the opportunity to develop the skills so that we get better. On my journey, I am becoming a better team leader. I am practicing my patience and my perseverance in my own learning so that I become that better learner that attracts, using the Law of Cause and Effect, that team of dream achievers, the team of business builders.

When that pressure is applied in our lives, patience and perseverance, is what we need in order to grow through that pressure point. Once we do grow and find different habits to allow us to get past that pressure point, to allow us to develop the skills, to learn those new things that we need to learn, we come to a better place and we can handle more pressure. We have new skills. We know something different. We react in a different away. We remain patient. We practice the perseverance and can step into success.

Epidemiology of Medicare abuse: the example of power wheelchairs.

OBJECTIVES: To determine the effect of neighborhood ethnic composition on medical scooters prescriptions. DESIGN: The 5% noncancer sample of Medicare recipients in the Surveillance, Epidemiology and End Results (SEER)-Medicare linked database, from 1994 to 2001. SETTING: SEER regions. PARTICIPANTS: Individuals covered by Medicare living in SEER regions without a cancer diagnosis. MEASUREMENTS: Individual characteristics (age, sex, ethnicity, justifying diagnosis, and comorbidity), primary diagnoses, neighborhood characteristics (percentage black, percentage Hispanic, percentage with <12 years education, and median income), and SEER region. RESULTS: The rate of power wheelchair prescriptions was 33 times greater in 2001 than in 1994, with a shift over time from justifying diagnoses more closely tied to mobility impairment, such as strokes, to less-specific medical diagnoses, such as osteoarthritis. In multilevel, multivariate analyses, individuals living in neighborhoods with higher percentages of blacks or Hispanics were more likely to receive power wheelchairs (odds ratios=1.09 for each 10% increase in black residents and 1.23 for each 10% increase in Hispanic residents) after controlling for ethnicity and other characteristics at the individual level. CONCLUSION: These results support allegations that marketers promoting power wheelchairs and handicap scooters have specifically targeted minority neighborhoods

As I mentioned before, youve got a life. Youre a busy person. We all are. We live in a fast-paced society, the information age, where we have so many tasks to complete and so little time to complete them.
